Winston-Salem, NC -- An autopsy report confirms two people died at Forsyth Medical Center Saturday night from gunshot wounds to the head in a murder-suicide.
Winston-Salem Police Department said they seized a .380 caliber handgun from Tazeman Boone, which he used to kill Saqueena Cutchin before shooting himself.
Cutchin was a patient at the hospital at the time.

Cutchin and Boone had a history of domestic violence, according to the police department.
Police said they spoke with Cutchin multiple times and offered her counseling and assistance, but she declined.
There were four incidents filed with Winston-Salem police involving the two.
- 12-11-2012 Mr. Boone was charged with Assault with a Deadly Weapon and Battery of an Unborn Child. This was the first documented incident between the two parties. The case was pending in court.
- 12-25-12 Mr. Boone reported being threatened by Ms. Cutchin. Mr. Boone alleged that Ms. Cutchin threatened to "shoot up" the residence he resided in at the time. The magistrates office found no probable cause and no charges were issued.
- 03-29-13 Ms. Cutchin reported being assaulted by Mr. Boone. Mr. Boone was alleged to have brandished a firearm at the time of this incident. The magistrates office found no probable cause and no charges were issued.
- 04-14-13 Ms. Cutchin reported that Mr. Boone threatened to kill her. Mr. Boone was charged in this case with communicating threats and the case was pending in court.
Police are still investigating this incident, but they said they are not looking for any additional suspects. They ask anyone with information to call Crime Stoppers at (336) 727-2800.
